Developmental Milestones at 9 Months
A boy of unknown age weighs 8.9 kg. He sits steadily without support, creeps forward on hands and knees, and pulls himself up on a chair leg to stand. He picks up a raisin with a crude thumb to finger grasp, says dada repeatedly but toward every adult, and enjoys peek-a-boo. He does not cruise, wave bye-bye, or use specific words. Which age best matches this developmental profile?
- A.6 months
- B.9 monthsCorrect
- C.12 months
- D.15 months
Explanation
Creeping, pulling to stand while holding furniture, a crude pincer grasp, and using mama or dada nonspecifically constitute the classic 9-month developmental cluster. Gross motor progression runs from unsupported sitting at 6 months, to crawling and pulling to stand at 9 months, to cruising and standing alone at 12 months, to independent walking by 15 months at the latest. Language mirrors this: babbling at 6 months, nonspecific mama and dada at 9 months, one to three specific words at 12 months, and 6 to 10 words at 18 months. Social play such as peek-a-boo and pat-a-cake also emerges at about 9 months, and the crude pincer grasp matures into a neat thumb to index pincer by 12 months. One caveat is that the CDC and AAP developmental surveillance checklists revised in 2022 set each milestone at the age by which about 75 percent of children achieve it, so they list pulling to stand at the 12-month visit, whereas the classic 50th percentile milestones used by Nelson and by licensing examinations place crawling and pulling to stand at 9 months. Either way this child is not 12 months old, because he neither cruises, waves bye-bye, nor uses a single specific word. A 6-month-old sits but cannot creep or pull to stand. A 15-month-old walks independently, says 3 to 6 words, scribbles and stacks two cubes. Any child who is not walking independently by 18 months requires evaluation, including creatine kinase measurement in boys to exclude Duchenne muscular dystrophy.
Why each option
- A.
- A 6-month-old sits without support and babbles but cannot crawl or pull to stand.
- B.
- Correct. Creeping, pulling to stand, a crude pincer grasp, nonspecific dada and peek-a-boo define 9 months.
- C.
- At 12 months the child cruises, stands alone, waves bye-bye and uses at least one word meaningfully.
- D.
- At 15 months the child walks independently, says 3 to 6 words, scribbles and stacks two cubes.
